Complete Home Care Services (CHCS) provides a range of services. Selected services are often based on a clinical needs assessment provided by a Registered Nurse (RN) and then ordered by a physician. Our intake team can help you decide the best level, frequency and duration or service to meet your individual needs.

Services Include:

Skilled Nursing

Private duty nursing with Registered Nurses (RNs) or Licensed Practical Nurses (
LPNs) to offer advanced care.
This may include disease care, wound care, medication management, catheter
care and patient education. Nurses are trained to provide the highest quality Tracheostomy care, Ventilator care, Respiratory treatments, Catheter and ostomy care, Gastrostomy (feeding tube) care, Nasogastric (N-G) tube care, Medication and injection administration.

Pediatric care, maternity or post-hospitalization care may require skilled nursing.

Complete Home Care Services caregivers specialize in infusion services. We work with the pharmaceutical company and infusion services team to deliver safe infusions at home.

Care may be scheduled in visits or shifts as needed by the individual.

Personal Care

Supervised by a Registered Nurse and provided by Certified Home Health Aides (HHAs) and Personal Care Aides (PCAs). Personal care includes companion care, safety care and support with activities of daily living. We offer hourly, Live-In, 24 hours round the clock care services, assistance with staffing needs while hospitalized, and companionship services for medical appointments and special events. Patient needs vary. Caregivers provide senior and elder care services, and Alzheimer’s & dementia care. They also support ambulatory adults who need some assistance to maintain a healthy and independent lifestyle.

  • Companion Care: In addition to personal care, Companion Care disrupts isolation and creates social opportunities for patient’s at home. Duties may include Grocery, Shopping, Meal Preparation, Special Dietary Needs Adhered to, Specially Trained in Kosher, Light Housekeeping, Laundry, Home Safety Supervision, Escorting to Doctor Appointments, Escorting to Social Outings, Medication Reminders, Companionship & Conversation, Accompanying on Walks, Outings and other Pleasant Events, Household Tasks, Personnel Errands, Linen changing while in or out of bed, Reading Together, Art Therapy, Music Therapy

24 Hour Live-in Care

Homemaking & Light Housekeeping

With an emphasis on safety and cleanliness, Home Health Aides ensure that individuals can stay at home with a degree of support. For example, fall risk ensures assessment and support can decrease falls caused by area rugs or slippers.

Errands and Transportation

Create opportunities for mobility, social interaction and medical appointments. It doesn’t matter the frequency. A trusted caregiver can be there for support on a schedule or on demand, based on need.

Hospital Discharge Transition

Hospital discharge is a huge milestone. After a hospitalization, the Complete Home Care Services team works with hospital staff, discharge planners, social worker and therapists to ensure a smooth transition home. Getting out of the Access-a-Ride to that first cup of tea at the kitchen table, can all be made easier and safer with support.

Meals & Nutrition

Healthy meal preparation is the foundation for a good diet and good health. Caregivers add a degree of safety and socialization to meal times.

Environmental Safety

Caregivers ensure that the environment is safe and meets the standards of a healthy home care experience.

Infection Control

Hand washing, PPE, frequent disinfecting and infection control practices are embedded in all layers of care at Complete Home Care services. We use a data driven approach to evaluate the efficacy of our infection control practices in the field.

Hospice Care

Hospice is a compassionate philosophy of care and an interdisciplinary set of services designed to support patients and families through the end-of-life process. Addressing the mental, physical, spiritual, and emotional needs of a patient and their family at the end of life, hospice focuses on pain relief and symptom management to provide dignity and quality of life at the end of life.
